Services are available 24 hours a day, seven days a week; patients are admitted within 24 hours of referral.
Keeping You Involved and Informed
A VNA clinician will visit you before you are discharged from the hospital to fully assess your needs and develop a specially designed in-home IV therapy plan in conjunction with your physician.
A plan of care will include your prognosis along with prescribed medications, type of infusion and expected duration of care. You and your caregiver will walk through each step of the plan and will be provided with written training material to ensure you clearly understand your therapy routine in preparation for daily living with as much independence as possible.
