Nurse Coordinator-Family Connects  

The Visiting Nurse Association is currently seeking a detail oriented, self-motivated RN who enjoys making a difference in the lives of others for our Family Connect Program. 

This employee provides Nursing Coordinator manages nurse home visitors and the administrative and community specialists who support the program. This position is responsible for aiding the manager in implementing the Family Connects Model with fidelity based on key performance indicators monitored monthly. Technical assistance will be provided by Family Connects International regularly to help achieve these goals. 

Typical duties include: 

  • Become fully trained in the Family Connects nurse home visiting protocol and be able to perform home visits as needed. Will adhere to the essential job functions of the home visiting nurse. 
  • Assists with planning, implementation and evaluation of Family Connects program. 
  • Provides leadership, guidance and consultation to the Parenting Support Team and works collaboratively to ensure practices which enhance quality client care and promote a customer focused environment. 
  • Contributes to the development and revision of program policies, procedures, program development, and grant opportunities alongside the Clinical Manager and Director. 
  • Implement quality assurance practices, including quarterly visits with each nurse home visitor to assess fidelity to the Family Connects Model.  
  • Perform a health and physical assessment of the birthing parent and infant.  
  • Respond to client needs through flexible scheduling of home visits. 
  • Establish and nurture relationships, partnerships, and coalitions with community resources and services that provide support to families in a timely and appropriate manner as referred by Family Connects nurses and that participate in the community system of care for young children and their families. 
  • Participating in nurse case conference meeting to provide individual-case consultation about community resources to support nurse home visitors as they connect families with community resources, as needed. 
  • Coordinate scheduling of all nurse assignments and manage schedule and client database 

 

Requirements: 

  • Active Nebraska or other RN licensure in a compact state 
  • Current Certified Basic Life Support 
  • Graduation from an accredited School of Nursing. BSN preferred. 
  • The ideal candidate will develop clinical practice guidelines, answer staff clinical questions, and collaborate with the agency’s medical director, when necessary.  
  • Valid Driver’s License and Auto Insurance coverage 
  • Must have your own reliable transportation
